Treatment of Penetrating Head or Spine Trauma

What are penetrating head and spinal traumatic injuries?

Penetrating injuries that affect the head and spine demand immediate surgical intervention. These types of injuries stemming from motor vehicle accidents, gunshot wounds, stab wounds, sports injuries, and physical assaults are on the rise. The urgency of surgical care cannot be overstated. Severe damage to the spinal cord can disrupt sensory signals that regulate muscle movement, underscoring the need for swift action.

The symptoms of traumatic spinal cord injuries depend on the nerves affected, but common sensory symptoms include the following:

  • Intense pain
  • A numb sensation
  • A pins and needles sensation
  • Weakness or loss of function of the legs and sometimes arms

While spinal cord injuries often cannot be cured surgically, early surgical stabilisation helps facilitate earlier mobilisation and rehabilitation. There is hope for recovery through long-term rehabilitation. Neuro-recovery rehabilitation, an active process that enables the spinal cord to develop new pathways, can restore full mobility. This message of hope is crucial for those with spinal cord injuries, as it underscores the potential for regaining functionality and independence and, ultimately, restoring their quality of life.

Traumatic penetrating head injuries arise from severe external damage to the head, which occurs from an impactful force or sudden blow to the head that pierces through the skull. As a result, a foreign body (knife, bullet or shrapnel) tears through the skull and penetrates the brain tissue.

Treatment for a traumatic brain injury involves minimising the severity of symptoms and stabilising the patient. As a result, surgery is performed to alleviate pressure in the skull and prepare for the resection of any remaining debris and blood clots if present. Another complication of penetrating traumatic brain injury is aneurysm formation which needs to be investigated properly and treated if present to prevent life threatening rupture. Debridement is a priority, as any loose bone or foreign body fragments are surgically resected. Furthermore, if fractures are present, surgical repair of the breaks in the skull is performed. Pain management is crucial in alleviating symptoms and reducing discomfort following surgical repair.
